When you read about a mission and press “A” on the controller to continue, it registers the button being pressed twice which causes you to automatically accept the mission.
This also happens after missions, for instance when a faction asks if you want to help them, it will automatically select “yes” because the button seems to register twice after pressing “continue”.
This is using a Xbox One wired controller on PC.
